1. Click on the Credit Card picture, and the payment form will open in a new window.

2. Insert the License Number 02246 in the space for the Provider's License Number.

3. Enter your name as it appears on your card and payment information. It should say Richard A Guild as person payment being submitted to.

4. Complete the form and click on the Submit Payment button at the bottom.

5. Print yourself a copy of the receipt that will appear on screen.
